Hospitalized Women Willingness to Pay for Inpatient Screening Colonoscopy

This study examined whether hospitalized women aged 50–75 would be willing to pay out of pocket toward an inpatient screening colonoscopy, even though outpatient screening is typically covered by insurance. Using a contingent valuation survey at Johns Hopkins Bayview Medical Center, researchers enrolled 312 women admitted to medicine services who were amenable to inpatient colonoscopy.<br /><br />About 48% of participants said they were willing to pay something in advance toward the test. Among these women, the mean willingness to pay was $178.41 (95% CI, $40.67–$316.16), suggesting that many hospitalized patients place meaningful value on receiving colorectal cancer screening during hospitalization.<br /><br />The analysis compared women who were willing to pay with those who were not. Most demographic and clinical factors were similar between groups, including age, race, education, income, primary care access, screening non-adherence, and colorectal cancer risk. Current smoking was the main factor associated with willingness to pay in the unadjusted comparison. In probit regression models adjusted for multiple factors, willingness to pay remained significant.<br /><br />The authors concluded that many hospitalized women value the opportunity for inpatient screening colonoscopy and may support integrating prevention into acute care. They suggest future research should assess feasibility, financing, and implementation, especially for hospitalized patients who are non-adherent with colorectal cancer screening and at higher risk.<br /><br />Key limitations included the single-hospital setting, the hypothetical nature of the willingness-to-pay question, possible framing effects, and no assessment of how inpatient screening might affect hospital staff or patients who might ultimately refuse the procedure.
