Hospitalists' Perceptions About Responding to Rapid Response Calls

This study surveyed hospitalists and advanced practice practitioners (APPs) at the University of Chicago Medical Center to understand their experiences responding to rapid response team (RRT) calls and to identify educational needs. Of 75 eligible providers, 48 (64%) completed the anonymous survey; most respondents were hospitalists, and over half had no prior RRT experience.<br /><br />Overall, providers felt more confident managing and communicating about unstable patients when the patient was on their own service than when cross-covering. Specifically, 83.4% reported confidence managing unstable patients during RRT calls for their own patients, compared with 50% for cross-covered patients. Similarly, 89.4% felt they effectively communicated the clinical situation for their own patients, versus 48% for cross-covered patients. About 79.2% said they clearly understood their role during RRTs, but 50.1% reported at least some loss of autonomy over patient care to the RRT.<br /><br />Common reasons for reduced comfort during RRT calls included lack of goals-of-care documentation, unfamiliarity with cross-covered patients, and communication difficulties with the Medical Intensive Care Unit team. The study found no significant differences in confidence based on years of experience or prior RRT experience.<br /><br />The authors conclude that educational efforts should focus on improving cross-coverage patient management, restoring provider autonomy, and strengthening communication between care teams. They suggest that simulation-based onboarding for new staff could help address knowledge gaps, improve teamwork and leadership, and better prepare providers for RRT events.
