Hospitalist Perceptions and Utilization of Artificial Intelligence in Writing Letters of Recommendation

This cross-sectional survey studied hospitalist faculty perceptions and use of artificial intelligence (AI) in writing letters of recommendation (LORs). Forty-seven hospitalists at a single academic institution responded. AI use was mixed: 40% reported no use, while 36% had used AI at least once in the prior two years. The most commonly used tool was ChatGPT.<br /><br />When AI was used, it was mainly as an assistive tool rather than for full letter generation. Common uses included editing existing drafts, improving grammar and clarity, generating ideas or structure, and drafting portions of letters.<br /><br />Faculty saw several benefits of AI-assisted LOR writing. The most frequently cited advantages were time savings (77%), improved writing clarity or quality (72%), and help overcoming writer’s block (57%). However, concerns were substantial. The top concern was loss of personal authenticity (83%), followed by over-reliance on technology (53%), risk of inaccuracies (45%), and ethical or professional concerns (40%).<br /><br />Most respondents (74%) supported creating institutional guidelines for appropriate AI use in LOR writing. Overall, the study suggests that while AI is already being used by some hospitalist faculty to reduce writing burden and improve drafts, many remain cautious about preserving authenticity and professionalism. The authors conclude that clearer standards and further research are needed to guide ethical, transparent AI use in recommendation letters.
