Hospitalist Co-Management Improves Outcomes for Vascular Surgery Patients
Back to course
Pdf Summary
The document summarizes a study of a formal co-management agreement between Hospital Medicine and Vascular Surgery teams, implemented in November 2021. Researchers compared patient characteristics and outcomes during the 16 months before and after the agreement.

After the agreement, more vascular surgery patients were seen by hospitalists, increasing from 8% before to 22% after. Patients seen by hospitalists had higher rates of diabetes and acute kidney injury than those not seen, suggesting hospitalists were more often involved in sicker or higher-risk patients.

Despite similar demographics and comorbidity rates overall between the before and after periods, several outcomes improved after co-management began. The percentage of patients discharged directly home increased from 68.2% to 80.2%. Thirty-day readmissions fell from 26.9% to 14.5%, and in-hospital mortality decreased from 5.8% to 1.7%. These changes were statistically significant.

However, some outcomes did not change meaningfully. Length of stay remained similar, as did the frequency of acute kidney injury and glycemic control during hospitalization.

Overall, the study concludes that formal hospitalist co-management in vascular surgery was associated with increased hospitalist involvement and improved patient outcomes, especially higher home discharge rates, fewer readmissions, and lower mortality.
