This study examined COVID-19-related publications in the top four general medical journals—NEJM, JAMA, JAMA Internal Medicine, and Annals of Internal Medicine—from March 1, 2020 to February 28, 2021. The authors reviewed 596 articles and identified 3,189 authors, including 1,874 physicians and 1,315 non-physicians. Only 77 authors (2.4%) were hospitalists, while 1,797 were non-hospitalist physicians (56.3%).<br /><br />Among hospitalist authors, 19.5% were first authors, 63.6% were middle authors, and 16.9% were senior authors. Most hospitalist-authored papers were research articles (63.6%); fewer appeared as commentary/editorials/viewpoints (32.5%), and very few were clinical reviews or educational articles (3.9%).<br /><br />The study highlights a major publication disparity: although hospitalists were frontline clinicians during the pandemic and played key roles in caring for hospitalized COVID-19 patients, they were rarely represented among authors of COVID-19 publications in leading journals. The authors suggest that increased clinical demands during the pandemic may have further limited hospitalists’ scholarly productivity.<br /><br />The paper concludes that the pandemic did not appear to create substantial new opportunities for hospitalists to disseminate their clinical knowledge through publication. Instead, existing disparities in academic output likely persisted or worsened. The authors recommend further research into barriers and supports for hospitalist scholarship, as well as improvements in institutional research infrastructure and promotion criteria that recognize non-research contributions.
