Hospital and Transitional Care of Low Impact Vertebral Compression Fracture

Vertebral fractures are noted as common osteoporotic fractures that can cause pain, disability, reduced quality of life, lower survival, and increased health care costs.<br /><br />The study aimed to evaluate how these fractures were managed in the hospital and at discharge. Findings showed that inpatient care commonly included physical therapy, pain control, spine consultation, and TLSO bracing. However, osteoporosis was poorly documented: only 18% of patients with a new vertebral fracture had osteoporosis listed in their problem list or discharge paperwork. Although 20% had a prior osteoporosis diagnosis in the electronic medical record, only 2% had documented prior treatment.<br /><br />Discharge planning was also limited. No patients were referred for bone density testing or outpatient follow-up to consider osteoporosis medications such as bisphosphonates or teriparatide. The conclusion was that there are significant opportunities to improve both inpatient and transitional care for patients with vertebral compression fractures, especially by better recognizing and treating underlying osteoporosis.
