Hepatic Artery Pseudoaneurism-an Institutional Experience

This Mayo Clinic retrospective study reviewed hepatic artery pseudoaneurysm (HAP) cases across three sites from 1998 to 2022. Of 37 identified cases, 27 met inclusion criteria. The goal was to describe clinical features, causes, treatment, and outcomes, and compare results with older literature.<br /><br />HAP is a rare but potentially life-threatening vascular complication, especially after liver transplantation or other hepatobiliary procedures. Historically, it has been associated with very high mortality and infection rates, particularly in post-transplant patients. In contrast, this study found much better outcomes.<br /><br />Key findings:<br />- Overall mortality was 18.5%.<br />- Infection rate was only 3%, far lower than older reports.<br />- Most HAPs were related to prior procedures (89.9%), including TIPS/balloon angioplasty, liver transplant, liver biopsy, ERCP/biliary drainage, cryoablation, Whipple procedure, laparoscopic cholecystectomy, and trauma.<br />- 11.1% were spontaneous.<br />- Common associated conditions included malignancy, cirrhosis, anticoagulation use, and liver transplant history.<br /><br />Risk factors linked to worse 1-year survival were:<br />- Tumor invasion<br />- Liver necrosis<br />- Portal vein thrombosis (PVT)<br />- Blood transfusion<br /><br />Notably, factors such as male sex, anticoagulation, GI bleeding, rupture, and liver transplant history were not significantly associated with survival.<br /><br />The authors conclude that improved imaging, earlier diagnosis, and better management at tertiary academic centers likely explain the lower mortality and infection rates compared with historical data. HAP remains important because it can rupture and cause bleeding, but mortality appears lower than previously reported when detected and treated promptly.
