This document describes an innovative onboarding “boot camp” for newly hired nocturnist nurse practitioners (NPs) joining a hospitalist program at a large academic medical center. Because there is no standard approach for onboarding advanced practice providers, the team designed a condensed orientation to improve NP readiness for overnight cross-coverage.<br /><br />The onboarding process included shadowing hospitalists, ICU APPs, and the rapid response team, followed by a half-day boot camp. The boot camp had two parts: general education and one-on-one clinical scenario review. The general education covered billing, general management, and electronic medical record tips. The clinical cases focused on five common acute issues: chest pain, gastrointestinal bleeding, altered mental status, sepsis, and atrial fibrillation with rapid ventricular response.<br /><br />Participants completed pre- and post-boot camp surveys measuring overall comfort in the role, confidence managing acute situations, knowing when to escalate care, and self-rated knowledge of the five scenarios. They also answered multiple-choice vignette questions.<br /><br />Six NPs have completed the program so far. Results showed positive feedback overall, with improvements in comfort managing acute care, recognizing when to escalate, and perceived expertise in the five clinical areas. These gains were supported by better scores on vignette questions. Statistically significant improvement was seen in comfort managing acute situations and knowledge of gastrointestinal bleeding.<br /><br />The authors conclude that a condensed NP orientation can effectively prepare APPs for nocturnist cross-cover roles. They note that more data are needed to assess long-term retention and satisfaction compared with other onboarding models.
