Healthcare Providers Knowledge, Attitudes and Perceptions on Pre-Exposure Prophylaxis (Prep) for Hiv

This document summarizes a study on healthcare providers’ knowledge, attitudes, and perceptions of HIV pre-exposure prophylaxis (PrEP) at Lincoln Hospital in the South Bronx. The aim was to explore barriers to PrEP prescribing and identify ways to improve uptake in a borough with one of the highest HIV rates in New York City, where PrEP use remains far below need.<br /><br />From January to July 2024, anonymous SurveyMonkey questionnaires were distributed to ambulatory clinic providers. The survey assessed demographics, knowledge, attitudes, perceptions, and prescribing experience. Ninety-one providers participated: 72 residents and 19 attendings. Data were analyzed using SPSS, including chi-square tests and binary logistic regression.<br /><br />Key findings showed that attendings were more likely to prescribe PrEP than residents. Among non-prescribers, common concerns included cost/insurance issues, medication non-compliance, and riskier sexual behaviors. Insufficient education on PrEP was the main concern for 71% of non-prescribers. Factors most likely to encourage prescribing included patients asking about PrEP, more training and education, a clear prescribing protocol, and pharmacy support with insurance coverage.<br /><br />The study found that perceived knowledge had a stronger influence on prescribing behavior than actual knowledge. Education increased confidence in prescribing PrEP even when providers lacked full knowledge. Differences between prescribers and non-prescribers also revealed misconceptions that may limit prescribing.<br /><br />Overall, the study concludes that targeted education, improved provider confidence, better patient-provider communication, and practical prescribing support are needed to increase PrEP use in the Bronx and address HIV prevention gaps.
