Healthcare Disparities Not Found in Hospital-Based Asthma Intervention Implementation

This study examined whether a hospital-based, systems-level asthma care bundle was implemented differently across patient groups, especially among underserved populations. The analysis used data from a prospective multicenter study of 100 adults aged 18–54 years hospitalized with acute asthma. The intervention bundle included laboratory testing, asthma education (action plan, inhaler technique, peak flow meter, smoking cessation help), and transitional care steps such as corticosteroid prescriptions, inhaled steroid adjustments, and scheduling follow-up with an asthma specialist.<br /><br />Researchers assessed overall bundle implementation using a composite concordance score and also looked at individual components, including action plan provision, inhaler education, and follow-up after discharge. Patient characteristics examined included age, sex, race/ethnicity, education, smoking status, socioeconomic status, insurance, primary care access, and comorbidities. Generalized linear mixed models were used for analysis.<br /><br />The main finding was that there were no significant disparities in delivery of the overall intervention bundle or any individual bundle element across patient subgroups. The median bundle implementation score was 6.3 out of 10. The sample was mostly female (72%), Black (66%), with 12% identifying as Hispanic, and the median age was 40 years.<br /><br />The authors conclude that standardized, systems-level bundled asthma interventions may help reduce the influence of provider bias and promote more equitable care delivery. However, the study was limited by its small sample size, focus on mostly severe asthma patients from urban settings, and lack of provider-level data. Further research is needed to determine whether such bundled interventions can improve asthma outcomes and reduce disparities across diverse populations.
