This ecological study used Global Burden of Disease 2019 data to examine trends in kidney dysfunction burden at global, regional, national, and SDI levels from 1990 to 2019. The authors analyzed summary exposure values (SEVs), deaths, and disability-adjusted life years (DALYs), using estimated annual percentage changes to assess trends over time.<br /><br />Globally, the absolute burden increased substantially over the 30-year period. Deaths attributed to kidney dysfunction rose from 1.57 million in 1990 to 3.16 million in 2019. Overall, SEV increased by 18%, deaths by 101%, and DALYs by 82%. However, age-standardized rates told a different story: age-standardized deaths and DALYs declined slightly, while age-standardized SEV increased modestly. This suggests that population growth and aging likely contributed to the rising total burden, even as standardized rates improved.<br /><br />By SDI level, high and high-middle SDI regions showed clear decreases in death and DALY burden, indicating progress in prevention and management. In contrast, middle and low-middle SDI regions experienced increases or minimal change in burden, while low SDI regions showed small declines in deaths and DALYs.<br /><br />The study concludes that the global age-standardized burden of kidney dysfunction has remained relatively stable over the past three decades, but trends differ by development level. The findings highlight the apparent effectiveness of health policies in high-SDI countries and the need for stronger strategies in lower-SDI settings to reduce the growing burden of kidney dysfunction.
