Nurses document what matters most to patients each day, pharmacists review high-risk medications and recommend deprescribing when appropriate, clinicians screen daily for delirium using BCAM to support early intervention, and mobility is assessed with AM-PAC scores to encourage early movement and preserve function. Hospitalists, nurses, pharmacists, and physical therapists collaborate in real time to align care plans with patient goals and geriatric priorities.<br /><br />The initiative was designed to ensure consistent, patient-centered, high-value care for older adults. Reported outcomes from 2020–2025 suggest sustained improvement after implementation, including a decrease in excess days, a 12% readmission rate in 2025, a 10-point increase in HCAHPS scores, and achievement of a CMS 5-star rating.<br /><br />Overall, the project argues that integrating the 4Ms into daily hospital workflows is a practical and scalable strategy for improving geriatric care, supporting better outcomes, stronger patient experience, and improved operational performance.
