This study examined gender differences in academic authorship among hospitalists during the first year of the COVID-19 pandemic (March 2020–February 2021). Researchers reviewed COVID-19-related articles in four major general medical journals: NEJM, Annals of Internal Medicine, JAMA, and JAMA Internal Medicine. They included papers with US-based physician first or last authors and classified authors by specialty and gender using online biographies or an automated gender tool.<br /><br />A total of 596 articles and 1,871 US-based physician authors were analyzed. Women represented 35.8% of all physician authors, but only 27.3% of hospitalist authors. This underrepresentation was especially noticeable in senior author positions. Disparities for women hospitalists were larger than those seen in other specialties. The gap appeared in both research articles and non-research articles; notably, there were no women hospitalists among senior authors of non-research pieces.<br /><br />The authors suggest that the pandemic may have worsened existing inequities because women hospitalists likely faced extra clinical and non-clinical responsibilities during this period. These added burdens may have reduced time and opportunity for research and publication.<br /><br />The study concludes that hospitals and academic departments should recognize the disproportionate impact of the pandemic on women hospitalists. Institutions should create policies and support systems that help women hospitalists conduct research, publish their work, and advance toward senior authorship, especially since publication is often tied to promotion and career advancement.
