Gender Disparities in Clinical Outcomes of Acute Pancreatitis: A National Cohort Analysis

This poster reports a nationwide retrospective analysis of acute pancreatitis (AP) outcomes in males versus females using the 2020 Nationwide Inpatient Sample (NIS) database and ICD-10 codes. The study included 252,595 adult hospitalizations for AP, with a mean age of 50.9 years; 55.1% were male and the majority were White (63.64%).<br /><br />The primary outcome was in-hospital mortality, and secondary outcomes included length of stay (LOS), hospitalization charges, acute kidney injury (AKI), sepsis, shock, and ICU admission. Multivariate logistic regression was used to adjust for confounders and determine whether sex was an independent predictor of outcomes.<br /><br />Female patients had a higher prevalence of congestive heart failure, chronic kidney disease, and obesity, while males had more type 2 diabetes. After adjustment, females had significantly better inpatient outcomes than males, including lower odds of in-hospital mortality (aOR 0.64), AKI (aOR 0.72), sepsis (aOR 0.68), and shock (aOR 0.74). These findings indicate that females had 36% lower odds of death, 28% lower odds of AKI, 32% lower odds of sepsis, and 26% lower odds of shock compared with males.<br /><br />There were no statistically significant sex-based differences in LOS, total hospitalization charges, or ICU admission rates.<br /><br />Overall, the study concludes that female patients hospitalized with acute pancreatitis experience significantly better clinical outcomes than male patients. The authors suggest that further prospective research is needed to better understand the reasons for these differences and to inform clinical practice.
