Gastrointestinal Pathogen Panel Study in Hospitalized Adult Patients

This retrospective chart review examined hospitalized adults who underwent gastrointestinal pathogen panel (GIP) testing at Mayo Clinic affiliated hospitals between May 5, 2018, and January 7, 2023. The study aimed to understand clinical factors associated with test ordering, the impact of results on patient care, and how GIP compares with other diagnostic approaches for gastrointestinal infections.<br /><br />A total of 6,053 GIP tests were performed in 5,786 patients. Positive results occurred in 2,129 tests across 1,862 hospitalizations. The most frequently detected pathogen was Clostridium difficile toxin (1,070 positive tests, 50.3%), followed by norovirus GI (297, 14.0%) and enteropathogenic Escherichia coli (EPEC) (277, 13.0%). The remaining 18 organisms accounted for 485 positives (22.8%).<br /><br />The hospitalized cohort had a mean age of 62.3 years, with slightly more women than men. Most patients were White (93.7%), and most were discharged from Internal Medicine services (82.4%). In the positive-result subgroup, fever (>100.4°F) was present in 649 cases (34.9%), and acute kidney injury in 119 (6.4%). Internal Medicine services also ordered the most tests and had the most positive results, followed by Transplant services.<br /><br />Overall, the study suggests that GIP testing is widely used in hospitalized adults with suspected gastrointestinal infection. Clostridium difficile was the dominant finding, highlighting its importance in healthcare settings. Detection of pathogens such as E. coli and norovirus shows the broader diagnostic value of the panel. Fever appeared to be associated with positive results, indicating it may help guide initial clinical assessment.
