Fostering the Development of Adaptive Expertise in ...
Fostering the Development of Adaptive Expertise in Hospital Medicine
Back to course
Pdf Summary
The document describes a Mayo Clinic project aimed at fostering adaptive expertise in hospital medicine education. The core problem is that residents rotating through hospital medicine must not only build broad clinical knowledge, but also learn how to apply it in complex, uncertain, or poorly defined situations where routine knowledge is not enough. To address this, the curriculum is designed to help residents become master adaptive learners. The initial approach included a “**reflective pause**” during rounds to encourage learners to think about what they needed to learn when faced with unclear cases. However, the pilot showed that busy workflow made reflection on rounds difficult, and residents often struggled to identify knowledge gaps on their own. Supervisors were helpful when they reframed learner uncertainty. Based on these findings, the next phase of the curriculum expands into a more structured approach with: - Daily reflection on uncertainty - Classification of uncertainty - Weekly reflection on uncertainty management and feedback - Didactic and experiential teaching The curriculum also includes efforts to study how experienced hospitalists manage uncertainty through qualitative research, then incorporate those strategies into teaching. Assessment items focus on whether learners can recognize their own uncertainty, identify when existing knowledge is insufficient, and navigate situations without a clear accepted solution. Overall, the project aims to create a practical educational framework that helps residents develop the habits and skills needed to manage uncertainty effectively and grow into adaptive clinicians.
