Fostering Trainee Participation in Hospital Patient Safety Event Reviews

This poster describes an initiative at the University of Washington Medical Center to increase trainee attendance at hospital patient safety event reviews. Based on ACGME CLER 2.0 guidance, the team aimed to raise trainee participation from 0% to 50% by December 2021.<br /><br />The project took place at a 529-bed academic medical center with about 1,100 trainees across 90 accredited programs. To understand why participation was low, the team interviewed stakeholders and identified several barriers: GME leaders did not know how to access event review opportunities, communication between Graduate Medical Education and the Patient Safety Office was unclear, and trainees had busy, variable schedules across multiple sites.<br /><br />In response, the team created a direct invitation process. The Patient Safety Office notified GME administration of scheduled event reviews, GME then contacted program directors for trainees on the relevant service, and invited trainees connected directly with the Patient Safety Office for details and attendance coordination. This process began in December 2019. In March 2020, all event reviews moved to virtual format because of the COVID-19 pandemic.<br /><br />The intervention improved access to reviews and increased participation over time, though exact quarterly percentages are shown in the poster figure rather than the text. The authors conclude that direct collaboration between patient safety leadership and GME was essential, and that virtual participation likely made attendance easier. However, they note that 100% participation remains difficult because of demanding clinical schedules, the multi-site nature of the institution, and short notice for some reviews. Smaller programs also have fewer opportunities because attendance depends on where trainees rotate.<br /><br />Future plans include opening attendance to trainees not currently rotating on the involved service and studying the educational impact of live event review participation.
