Foley Removal Strategy: Optimizing Catheter Use and Outcomes

This study evaluated a “Foley Removal Countdown Strategy” designed to reduce unnecessary indwelling urinary catheter use in a hospitalist ward and improve patient safety. Indwelling urinary catheters can help with care but also increase the risk of catheter-associated urinary tract infections (CAUTIs) and other complications, so earlier removal is often desirable.<br /><br />The prospective cohort study included 80 patients in a tertiary care hospital. Researchers collected data on patient characteristics, reasons for catheter use, catheter duration, complications, and length of stay. The intervention required daily assessment of whether a catheter was still needed, with the goal of prompting timely removal.<br /><br />Results showed that the strategy was effective. The catheter removal rate reached 65.2%, exceeding the target and representing a substantial improvement over prior practice. Average catheter duration fell from 21 days before the intervention to 13.1 days afterward. Catheter indwelling rate also decreased slightly, from 28% to 25.33%, and infection rates dropped from 3.07% to 2.59%.<br /><br />Overall, the Foley Removal Countdown Strategy appears to be a practical and effective approach for reducing catheter use, lowering CAUTI risk, and potentially decreasing other catheter-related complications and hospital stay length. The authors suggest that further validation in larger multicenter studies and randomized controlled trials is needed.
