Factors Associated With Increased Length of Hospital Stay in Patients With Covid-19: Results From the American Heart Association National Registry

This study used the American Heart Association National COVID-19 Cardiovascular Disease Registry to identify factors linked to longer hospital length of stay (LOS) among adults hospitalized with COVID-19 from March to November 2020. The analysis included 20,793 patients admitted to more than 130 hospitals across 30 states.<br /><br />Overall median LOS was 6 days, and LOS declined over time during the study period. Patients hospitalized earlier in the pandemic had longer stays, while the proportion of patients in the longest-stay group decreased from March–May to September–November 2020. Mortality also varied by LOS, increasing among patients with longer hospitalizations.<br /><br />Several factors were associated with longer LOS in multivariable analysis. Older age, higher respiratory rate at admission, transfer from another hospital, and comorbidities such as malignancy, diabetes, cerebrovascular disease, respiratory disease, cardiac disease, and chronic kidney disease were linked to increased LOS. Patients with Medicare, Medicaid, or other government insurance also had longer stays than those with commercial insurance.<br /><br />In-hospital complications had the strongest associations with prolonged LOS. New thrombotic events, new need for dialysis or renal replacement therapy, and intubation were all associated with substantially longer hospitalizations, increasing LOS by roughly 5 to 10 days. Larger hospitals had slightly longer LOS than small hospitals, while urban hospitals had somewhat shorter LOS than rural hospitals. Regionally, LOS was longest in the West.<br /><br />Female sex and obesity were associated with slightly shorter LOS. The authors conclude that multiple patient, hospital, and geographic factors predict prolonged hospitalization in COVID-19 and that these findings may help hospitals plan resources during future surges or pandemics.
