This multicenter prospective observational study examined factors affecting clinician awareness of peripheral intravenous catheters (PIVs) in hospitalized, non-critically ill patients at four Michigan hospitals. PIVs are widely used but can cause serious complications, and clinician awareness of their presence may influence care. Researchers interviewed 1,682 patients to confirm whether they had a PIV, then asked 4,961 care providers—nurses, attending physicians, residents, and medical students—whether their patient had one. They also recorded whether patients had multiple PIVs, a central line, contact precautions, or a running infusion. Provider groups were compared using chi-square tests.<br /><br />Overall, 66.7% of patients had a PIV. Awareness varied by clinician type, with nurses generally more aware of PIV presence than physicians. Physician residents and attendings showed lower awareness when patients also had a central line than when they did not. Nurses had lower awareness in patients on contact precautions compared with those not on precautions. Nurse awareness was higher when an infusion was running.<br /><br />The study suggests that awareness of peripheral IVs is influenced by the presence of other devices or precautions, especially central lines and contact precautions. Because physicians order and manage IV access while nurses place and maintain it, differences in awareness may reflect workflow and role separation. The findings raise concerns that reduced awareness could affect patient safety and care quality, particularly in settings where multiple vascular devices are present or where contact precautions limit healthcare worker interaction.
