Expanding Treatment Access for Hospitalized Adults With Opioid Use Disorder

The SHOUT Texas project expanded hospital-based treatment for adults with opioid use disorder (OUD) by spreading a successful hospitalist-led buprenorphine program (“B-Team”) to additional Texas hospitals. The expansion used a combined implementation strategy: the Replicating Effective Programs (REP) framework for training, tailoring, and technical assistance, plus Project ECHO for statewide telementoring and ongoing education.<br /><br />From 2020 to 2023, the program grew to three additional hospital sites and resulted in 3,065 hospitalized adults starting medication for OUD (MOUD). Across 2021–2023, the number of patients screened for OUD, started on MOUD, and discharged with coordinated outpatient care increased substantially. More than 2,500 clinicians and care team members received SHOUT training, and 241 participated in at least one ECHO session. Training feedback was highly positive, with most respondents reporting that the overall quality was “very satisfied” or “satisfied,” and that the material was useful in practice.<br /><br />Key facilitators identified through stakeholder interviews included pre-packaged educational and resource materials, logistical support and implementation guidance, the SHOUT Texas network and learning community, and seed funding. Major barriers included challenges in securing outpatient follow-up, site-specific pharmacy and medication policies, limited nursing education, and difficulties integrating the model into other hospital units and disciplines.<br /><br />Overall, the project found that combining REP and Project ECHO enabled rapid expansion of inpatient MOUD access across diverse hospital settings. The findings suggest that this implementation approach can support wider adoption of OUD treatment in hospitals and provide lessons for sustainability, adaptation, and future growth.
