Evaluation of a Hospital Medicine Fellowship for Advanced Practice Providers

This poster evaluates a 12-month hospital medicine fellowship for advanced practice providers (APPs) at the University of Utah, started in 2017. The fellowship includes 9 months of hospital medicine and 3 months of inpatient subspecialty rotations, along with didactics, learning modules, and scholarly projects. APP fellows had lower clinical volumes than full-time APPs at first, with workload increasing over time, and were paid 30% less than full-time APPs.<br /><br />To assess the program, the authors surveyed 14 graduates; 12 responded (86%). Overall, responses were very positive. Most APPs agreed that the fellowship provided high-quality medical education, appropriate supervision, strong teaching, evidence-based medicine training, and opportunities to work at the top of their license. They also felt the fellowship was a good career decision and that it helped prepare them for practice, with many indicating that it increased autonomy over time. After graduation, APPs strongly wanted to join the hospitalist group.<br /><br />Areas for improvement were also identified. Feedback was generally given every three months, which half the respondents felt was “slightly too little,” suggesting that more frequent feedback may be beneficial. Another major gap was bedside procedures: APPs reported feeling inadequately trained in this area. The study suggests the fellowship is valuable and well-received, but could be strengthened by increasing feedback frequency and enhancing procedural training.<br /><br />Overall, the authors conclude that the APP hospital medicine fellowship was viewed as a worthwhile educational experience that supported career development and readiness for hospital medicine practice.
