Evaluating the Impact of Structured Interdisciplinary Rounds (Pocr) on Communication, Care Coordination, and Operational Efficiency in an Academic Rural Health System

A standardized Structured Interdisciplinary Rounds model (POCR) was implemented across several inpatient medical units in 2024 to improve communication, reduce care fragmentation, and support discharge efficiency. The model used a structured script, a unified flow board, and defined roles for nursing, case management, pharmacy, and providers.<br /><br />This study assessed early multidisciplinary perceptions of POCR one year after implementation using a cross-sectional anonymous survey distributed to all rounding participants. Forty-eight respondents participated: providers (20), case management (17), nursing (9), and pharmacy (2). The survey covered seven domains: nursing workflow support, pharmacy engagement, case management coordination, communication and collaboration, role clarity and efficiency, patient impact and safety, and operational friction.<br /><br />Results showed strong overall favorability, especially in communication and collaboration (83% favorable) and patient impact and safety (76% favorable). Participants felt POCR improved shared decision-making and clarified discharge planning, with 94% favorability for discharge criteria clarity. However, operational friction remained a concern, with 27% favorable on that domain. The main barriers were crowding during rounds and bottlenecks related to order placement and transport coordination.<br /><br />Overall, respondents perceived POCR as beneficial for interdisciplinary communication and discharge coordination, while highlighting the need to improve workflow efficiency and reduce operational barriers.
