Evaluating the Impact of Audit and Feedback Using a Novel Text and Web-Based Platform With Unblinded Peer Comparison

This project evaluated whether automated audit-and-feedback using a text and web-based platform with unblinded peer comparison data could improve hospitalist practice. Over six months, 57 hospitalists at two large teaching hospitals received weekly automated text messages linking to individualized performance data. Each week focused on one process metric—daily labs, CT scans, echocardiograms, consultations, or early discharge orders—repeated three times during the intervention. Engagement was measured by whether hospitalists clicked the link to view their data and peer comparisons.<br /><br />The platform achieved strong engagement, suggesting that clinicians were receptive to the automated feedback approach. Results showed improvement in some process measures, including an increase in discharge orders placed before 10 a.m. and a decrease in CT utilization. However, there was no observed change in broader outcome measures such as length of stay or 30-day readmission rates.<br /><br />The study concluded that automated peer comparison can effectively influence certain physician behaviors and may be a scalable way to deliver audit-and-feedback interventions without requiring extensive manual effort. Based on feedback from participants, the next phase will refine the process metrics to better align with evidence-based pathways and metrics with clearer desired directionality.
