Evaluating Drug-Drug and Drug-Gene Interactions in Hospitalized Patients in a Tertiary Academic Center

This study evaluated drug-drug interactions (DDIs) and drug-gene interactions (DGIs) in recently hospitalized patients seen after discharge at a Personalized Therapeutics Clinic (PTC) at the University of Chicago. Hospitalized patients are especially vulnerable to medication-related problems because inpatient care is often fragmented, medication lists may be inaccurate, and newly started drugs can create DDIs or DGIs.<br /><br />Adults enrolled in the PROTECT-Rx trial were eligible if they were taking at least five medications with a significant inducer/inhibitor, or if they were taking a medication with potentially actionable pharmacogenomic information. Participants received broad pharmacogenomic testing at no cost and a one-time virtual consultation with a clinical physician pharmacologist, who reviewed their medications and provided recommendations to patients and their treating providers. Medication use and interaction rates were reassessed at 3 months.<br /><br />The analysis included 31 patients, with a mean age of 53.8 years; 80% were female and 67.7% had Medicare or Medicaid insurance. At baseline, 74.2% had clinically significant potential DDIs and 64.5% had cautionary or high-risk DGIs. Recommendations were made for 87.1% of patients for DDIs and 67.7% for DGIs.<br /><br />By 3 months, 21% of DDIs persisted, 21% were resolved by stopping one or more interacting drugs, 7% were resolved by switching to an alternative medication, and 50% were addressed through dose adjustment. For DGIs, 59% showed full alignment with the clinic’s prescribing recommendations.<br /><br />Overall, clinically significant DDIs and DGIs were very common in this hospitalized population, and the post-discharge PTC visit led to meaningful prescribing changes in many cases. The authors conclude that this approach shows promise for preventing adverse drug events and should be tested in a randomized study.
