Establishment of a Clinical Case Repository to Facilitate Case-Based Scholarship

This abstract describes the development of a clinical case repository designed to support case-based scholarship among hospital medicine trainees and faculty. The motivation was that, although hospital medicine offers many interesting clinical cases, scholarly dissemination is often limited by lack of time, mentorship, experience, or an organized system for identifying cases. The section previously had no standardized way to collect and store inpatient cases for future educational use.<br /><br />The team created a secure, web-based repository compliant with HIPAA standards and maintained by a multidisciplinary case-based scholarship group including medical students, a resident, an advanced practice nurse, and hospitalists. Cases were collected through sign-out, direct outreach, and workroom discussions, then stored on a shared patient list and reviewed for clinical relevance, key teaching points, and possible presentation venues. Trainees and hospitalists were matched based on interest and availability to develop the cases into scholarly products such as clinical vignettes, case reports, morning report presentations, podcasts, instructional images, and CME lectures.<br /><br />Since implementation, the repository has documented 53 submitted cases. These led to 38 clinical vignette submissions, 4 CME lectures, 3 published case reports, and 1 resident-led morning report. The authors conclude that the repository was highly effective in increasing case-based scholarship and provided educational and professional benefits for both trainees and hospitalists. Future goals include expanding case report development, broadening educational uses, and evaluating the repository as a pathway for trainees to enter scholarship.
