Escaping the Or: A Pilot Study of a Jigsaw-Based Workshop to Teach Preoperative Assessment in Internal Medicine Residency

This pilot study evaluated a structured, active-learning workshop using the Jigsaw Teaching Method to improve internal medicine residents’ knowledge and confidence in preoperative assessment. The need for the intervention arose from a needs assessment showing that preoperative evaluation was important but inconsistently taught in residency, especially in ambulatory training.<br /><br />The workshop was a 2-hour session for primary care track internal medicine residents in May 2025. It covered four key domains: cardiovascular risk assessment, medication management, anticoagulation, and special populations. Residents first became “experts” in one domain using self-study materials, then worked in small groups to apply their knowledge to two complex patient cases. The session concluded with a faculty debrief and a gamified escape-room-style activity called “Escape the OR.” A control group of categorical residents received standard clinical and didactic training.<br /><br />Pre- and post-intervention surveys measured both knowledge and confidence. Results suggested the intervention improved outcomes more consistently than standard training. In the intervention group, the largest knowledge gain was in the special populations domain, where correct responses increased from 55% to 96%. Cardiovascular risk knowledge also improved notably, while medication management and anticoagulation changed only modestly. Confidence scores increased across all domains in the intervention group. In contrast, the control group showed smaller, less consistent improvements in knowledge and confidence, with several confidence scores declining after training.<br /><br />Overall, the study suggests that a single, structured Jigsaw-based workshop can meaningfully improve internal medicine residents’ knowledge and confidence in preoperative assessment. The authors conclude that this peer-led, interactive approach may be a scalable model for addressing curricular gaps in ambulatory medical education.
