Epidemiology of Non-Variceal Upper Gastrointestinal Bleeding in Patients With Cirrohosis

This population-based retrospective study examined non-variceal upper gastrointestinal bleeding (NVUGIB) among cirrhotic patients using the U.S. National Inpatient Sample from 2007–2018. The authors analyzed 9,804,819 cirrhosis-related hospitalizations and identified 160,194 NVUGIB admissions (1.6%). The overall hospitalization trend remained stable over time, decreasing only slightly from 2.0% in 2007 to 1.7% in 2018, with no significant trend.<br /><br />Patients hospitalized with NVUGIB were slightly younger than other cirrhotic inpatients (57 vs. 59 years) and more often male (64%). In multivariable analysis, higher odds of NVUGIB were associated with older age, male sex, residence in the Western U.S., Helicobacter pylori infection, and alcoholism. Other factors linked to increased NVUGIB hospitalization included hypertension, valvular heart disease, NSAID use, and smoking. Several of these are potentially modifiable risk factors.<br /><br />The study also found important adverse outcomes. Among cirrhotic patients admitted with NVUGIB, 14.7% required discharge to a facility, 5.0% died during hospitalization, and the average length of stay was 6 days.<br /><br />Overall, the study demonstrates that NVUGIB is a significant and persistent cause of hospitalization in cirrhotic patients and is associated with substantial morbidity and mortality. The findings highlight the need for further research to identify causal mechanisms and develop preventive strategies in this high-risk population.
