Epidemiological Trends and Disparities in Renal Complications Among Adults With Diabetes Mellitus in the United States: A CDC Wonder Database Analysis (1999-2020)

This CDC WONDER database study examined mortality from renal complications of diabetes among U.S. adults aged 35–85 from 1999 to 2020. Using death certificate data, the authors calculated age-adjusted mortality rates (AAMR) and annual percent change (APC), stratified by sex, race/ethnicity, year, and region.<br /><br />The study identified 525,804 deaths related to diabetes-associated renal complications during the study period. Overall mortality increased sharply, with AAMR rising from 1.6 in 1999 to 34.9 in 2020, reflecting a marked upward trend.<br /><br />Important disparities were found. Men had a higher overall AAMR than women (17.8 vs. 11.2). However, women showed a large increase by 2020, reaching 44.2 compared with 27.6 in men. Among racial and ethnic groups, American Indian/Alaska Native adults had the highest overall AAMR (36.1), indicating the greatest burden in this population. Geographic differences were also evident: nonmetropolitan areas had higher mortality rates than metropolitan areas (16.0 vs. 13.5), and Western states were noted to have consistently higher rates.<br /><br />The findings show that mortality from renal complications of diabetes has risen substantially over time and is unevenly distributed across sex, race/ethnicity, and geography. The authors conclude that targeted public health interventions, improved healthcare access, and focused strategies for high-risk populations are urgently needed to reduce this growing burden.
