Epidemiological Trends and Disparities in Alcohol-Related Mortality Among Adults in the United States: A CDC Wonder Database Analysis (1999-2020)

This study examined U.S. alcohol-related mortality from toxic effects of alcohol (ICD-10 T51) among adults aged 25 and older between 1999 and 2020 using CDC WONDER death certificate data. Joinpoint regression was used to assess trends in age-adjusted mortality rates (AAMR) and average annual percent change (AAPC).<br /><br />Overall, toxic effects of alcohol caused 179,492 deaths during the study period. The national AAMR rose substantially from 1.04 in 1999 to 8.76 in 2020, with a significant AAPC of 10.04. Men had a higher overall AAMR than women (6.22 vs 1.97), but women experienced a slightly faster increase over time (AAPC 10.8 vs 10.2). By race and ethnicity, American Indian populations had the highest mortality rate (15.13), while Asian or Pacific Islander populations had the lowest (0.70). Non-Hispanic Black adults showed the highest trend increase over time (AAPC 11.5).<br /><br />Geographic differences were also notable. The Midwest had the highest AAMR (4.6), while the South had the lowest (3.5). All regions showed significant increases, with the fastest rise in the Midwest (AAPC 14.2). Metropolitan and nonmetropolitan areas had very similar rates overall. Most deaths occurred at decedents’ homes, followed by other/unknown locations. States in the highest mortality range included New Mexico, West Virginia, Alaska, Rhode Island, and the District of Columbia.<br /><br />The authors conclude that alcohol-related mortality has increased sharply across the U.S., with important disparities by sex, race, and region. They emphasize the need for targeted public health interventions to reduce alcohol-related harm.
