Enhancing Mortality Risk Documentation With Michart Clinical Assistance and Support Tool: A Novel Electronic Medical Record Smart Phrase Solution

This quality improvement project evaluated a new EMR Smart Phrase called MiCast, introduced in January 2024 at an academic medical center to improve documentation of mortality risk diagnoses in hospital medicine notes. MiCast automatically detects 24 common abnormalities and physician actions from test results and orders, then inserts approved text into clinical notes. These diagnoses include conditions such as kidney disease, blood disorders, electrolyte abnormalities, malnutrition, and obesity, all of which can affect mortality risk assessment and length of stay.<br /><br />The hospital medicine division launched a QI initiative in July 2024 to increase MiCast use and improve documentation quality. The strategy included provider training, a dashboard to monitor adoption, weekly chart review, email reminders, and monthly leadership updates. Providers reviewed the autogenerated text for accuracy before signing notes.<br /><br />Results showed a major increase in use of MiCast after the QI intervention. Before the project, MiCast appeared in 2.2% of notes (776 of 35,038) from January to June 2024. During the project, use rose to 67.0% of notes (38,811 of 57,439) from July 2024 to March 2025. The tool also helped capture additional mortality risk diagnoses in documentation, with 75% of notes before and during the project adding at least one such diagnosis.<br /><br />Overall, the project demonstrated that a newly implemented EMR Smart Phrase, combined with a comprehensive QI strategy, substantially improved adoption and enhanced mortality risk documentation across hospital medicine clinical services.
