Engaging Academic Hospitalists on Lab Stewardship in a Teaching Hospital

This project evaluated whether personalized feedback could reduce unnecessary lab ordering among LMC Academic Hospitalists after removing resident encounters from the data. On January 17, 2024, 15 of 26 hospitalists with individualized data received a personalized text message with a trackable link. The team planned to measure engagement, collect feedback through an email survey, and assess whether lab ordering changed significantly among those who viewed the data. They expected that only hospitalists on service that day would be likely to engage, so follow-up text reminders were also considered necessary.<br /><br />The results suggested mixed impact. For some Academic Hospitalists, the new data increased awareness of their practice patterns and was associated with reduced lab ordering. However, others did not notice the change, likely because they could not easily compare the new data with their prior data. This indicates that the presentation of feedback matters, especially for physicians with lower clinical FTE and fewer encounters. A feature allowing users to toggle between data with and without resident encounters may improve clarity, make trends easier to interpret, and address small-sample limitations. The study also concluded that additional text reminders are needed to reach hospitalists who were not on service during the initial intervention.<br /><br />Overall, the intervention showed promise as a stewardship tool, but improved data visualization and repeated outreach are likely needed to maximize engagement and effect.
