Electronic Health Record Efficiency Education in Residents

This Indiana University School of Medicine project studied whether targeted education could improve internal medicine residents’ efficiency and comfort using the electronic health record (EHR). Residents on a 4-week inpatient rotation were randomized to one of three groups: no added resource (control), a tipsheet, or an online course with video modules. Participants completed pre- and post-rotation surveys and provided qualitative feedback.<br /><br />Overall, comfort with EHR use improved in all groups. The intervention groups showed greater gains in certain areas: both the tipsheet and course improved chart review and admission/discharge tasks; the course group improved patient list management; and the tipsheet group improved pre-rounding and daily data collection.<br /><br />Residents described several major EHR challenges. The most common was information overload and interface complexity, with many saying there were “too many ways to do the same thing.” Other recurring issues included inefficient chart review and pre-rounding, complicated admission and discharge workflows, difficulty presenting and finding lab results, messaging overload, and uncertainty around templates and customization.<br /><br />The study concluded that EHR efficiency interventions were well received and may improve specific EHR-related skills. However, the feedback also highlighted broader system problems, especially information overload, suggesting that future improvements should address both education and EHR design.
