Effects of Anti-Diabetes Medications on Clinical Outcomes of Covid-19 Patients

This retrospective study from the Cleveland Clinic evaluated whether pre-admission diabetes medications affected COVID-19 outcomes in 909 hospitalized patients with diabetes. The medications of interest were metformin, DPP-4 inhibitors (DPP4i), and insulin, with outcomes including ICU admission, need for assisted ventilation, renal insufficiency, and mortality.<br /><br />The cohort had a mean age of 67.1 years, was slightly more male than female, and included a large proportion of Black and White patients. Common comorbid risk factors included obesity, chronic kidney disease, and high comorbidity burden. Medication exposure before admission was documented for at least 90 days.<br /><br />On univariate analysis, metformin was associated with lower mortality (p=0.039), while DPP4i showed no significant association with any outcome. Insulin use was associated with higher rates of ICU admission (p=0.001), need for assisted ventilation (p=0.004), and renal insufficiency (p=0.016).<br /><br />However, after multivariable adjustment for demographic factors, kidney function, diabetes type, HbA1c, smoking, insurance, comorbidity burden, and other medications, these associations were no longer statistically significant for most outcomes. Metformin was not significantly associated with ICU admission, ventilation, or mortality. DPP4i remained neutral across all outcomes. Insulin was independently associated with ICU admission in the adjusted model (odds ratio 1.96, 95% CI 1.40–2.76), but its associations with ventilation and mortality were not significant after adjustment.<br /><br />Overall, the study concluded that metformin did not show a confirmed protective effect after adjustment, DPP4i had no meaningful effect, and insulin use likely reflected greater illness severity or comorbidity rather than a direct harmful effect.
