Effect of Inpatient Breast Cancer Education and Risk Assessment on Mammography Utilization Among Hospitalized Women Post Discharge

This prospective interventional study examined whether inpatient breast cancer education and individualized risk assessment could improve mammography screening among hospitalized women aged 50–75 at an academic medical center. Women with prior cancer (except skin cancer) or serious conditions limiting life expectancy were excluded. The intervention consisted of bedside education using handouts and videos, plus Gail model risk assessment. Participants were followed by phone six months after discharge to assess screening adherence.<br /><br />The study found that about one third of eligible hospitalized women were not adherent to breast cancer screening guidelines. Nonadherent women were more likely to have lower income, be current smokers, have higher comorbidity burden, and have never previously had a mammogram. Although the intervention increased awareness and willingness—95% of adherent women and 77% of nonadherent women said they were willing to get a mammogram after the intervention—actual follow-through remained low. Among the small nonadherent group reached on follow-up, only 42% had obtained a mammogram after discharge, despite 92% seeing their primary care physician and 67% discussing screening with them.<br /><br />The main conclusion was that inpatient education and risk assessment were only partially successful. Awareness of breast cancer risk improved, but barriers to screening persisted after hospitalization. The authors suggest that future research should explore the feasibility of performing screening mammography during hospitalization for high-risk, nonadherent women.<br /><br />Limitations included a single-center design, reliance on self-reported data, poor follow-up response, and substantial refusal and loss before enrollment.
