Effect of Anticoagulants and Steroids on Covid Outcomes: International Study

This international retrospective cohort study compared COVID-19 outcomes in two healthcare settings: the University of Minnesota (UMN) in the United States and multiple hospitals in Cairo, Egypt. The study included 4,278 patients treated between March 1 and May 31, 2020, and examined whether differences in treatment strategies—especially corticosteroids (CCS) and anticoagulant therapy (ACT)—affected in-hospital mortality.<br /><br />The cohorts differed in case mix and treatment practices. US hospitals generally admitted more severe COVID-19 cases, while Egyptian hospitals admitted a broader range of symptomatic, COVID-positive patients under national management protocols. Researchers analyzed demographics, comorbidities, ICU admission, ACT, CCS, and mortality using chi-square/Fisher’s exact tests and multivariable regression.<br /><br />Key findings showed that mortality increased with older age, male sex, and ICU admission in both cohorts. In the Egyptian cohort, both anticoagulation and corticosteroid use were associated with significantly lower mortality. Specifically, age 75+, ICU admission, and male sex increased mortality, while ACT and CCS reduced it. In the UMN cohort, age 75+, male sex, and ICU admission also increased mortality, while hypertension was associated with lower mortality; anticoagulation showed a trend toward reduced mortality but did not reach statistical significance.<br /><br />Overall, the study concluded that both cohorts shared similar risk factors for death, but the Egyptian cohort showed reduced mortality with CCS and ACT, consistent with clinical trial evidence. Differences in the US cohort may have been due to lower event rates.
