Early-Career Hospitalists' Perspective on Hospital Medicine-Focused Education in Residency Training

This survey study examined early-career hospitalists’ (ECHs, 0–5 years post-residency) experiences and preferences regarding hospital medicine-focused education (HMFE) during residency. The authors noted that traditional residency training often does not fully prepare physicians for hospital medicine, and that most prior studies focused on residency leaders rather than trainees or early-career physicians.<br /><br />Using a validated survey distributed through the Society of Hospital Medicine, the study collected responses from 257 members, including 84 ECHs; 42 had participated in HMFE and 42 had not. Most respondents trained in internal medicine, with others from family medicine, med-peds, and pediatrics.<br /><br />Key findings showed that HMFE was highly valued: 89% of participants were satisfied with their experience. HMFE was associated with increased interest in hospital medicine (60%), better preparation for the first year as a hospitalist (76%), and improved readiness for academic hospital medicine (70%). It also modestly increased interest in academic hospital medicine overall.<br /><br />ECHs preferred HMFE experiences that included core hospitalist clinical work such as admitting new patients, seeing consults, triaging admissions, co-managing patients, and working on new units. They also valued non-clinical components like quality improvement and patient safety projects, teaching medical learners, and curricular content in evidence-based practice, career mentorship, high-value care, patient safety, and quality improvement.<br /><br />Participants identified major gaps in standard residency training, especially in career preparation, exposure to quality improvement/patient safety/high-value care, autonomy, academic hospitalist preparation, and help with the job application process. They also emphasized a need for hospital medicine faculty mentorship.<br /><br />The authors concluded that HMFE can bridge important gaps in both clinical preparation and career development for future hospitalists, particularly by offering specialized clinical experiences, mentorship, and autonomy that traditional inpatient wards may not provide.
