Does the Time From Discharge to Readmission Impact Outcomes?

This retrospective study used the National Readmission Database from 2010–2019 to examine 30-day readmissions after AMI, COPD, heart failure, pneumonia, CABG, THA, and TKA, and to determine whether the time between discharge and readmission affected outcomes. The analysis included 31.6 million hospital records, with 4.59 million (14.55%) readmitted within 30 days.<br /><br />Overall, readmission rates declined over time for COPD, heart failure, pneumonia, AMI, and TKA, while CABG and THA rates increased. Later study year was associated with fewer readmissions. Factors linked to higher readmission risk included male sex, teaching hospitals, and Medicare insurance, while smaller hospital size, higher income, and older age were associated with lower readmission likelihood.<br /><br />Compared with patients who were not readmitted, readmitted patients had higher mortality (6.0% vs 2.8%) and longer length of stay, though hospital charges were similar overall. Importantly, timing mattered: patients readmitted within 10 days of discharge had higher mortality than those readmitted at 11–20 days or 21–30 days. Longer time from discharge to readmission was associated with lower odds of death and lower hospital charges, but not a meaningful difference in length of stay.<br /><br />The authors concluded that readmission rates generally fell over the decade, but it is unclear how much the Hospital Readmission Reduction Program contributed because declines began before its implementation. The study suggests that early readmission is a marker of worse outcomes.
