Does Hospital Teaching Status Affect the Outcome of Sepsis? An Observational Study Using the Nationwide Inpatient Sample

This observational study examined whether hospital teaching status affects outcomes for adults hospitalized with sepsis using the U.S. Nationwide Inpatient Sample (NIS) from 2016–2017. The investigators identified 3,935,528 adult sepsis admissions and compared outcomes between teaching and non-teaching hospitals, adjusting for age, sex, race, income, insurance, and comorbidities.<br /><br />Most patients were admitted to teaching hospitals (62.5%), while 37.5% were admitted to non-teaching hospitals. Patients in non-teaching hospitals were slightly older on average. Overall inpatient mortality for sepsis was 9.7%.<br /><br />After adjustment, patients treated in teaching hospitals had worse outcomes than those in non-teaching hospitals. Specifically, teaching hospitals were associated with higher mortality (10.3% vs 8.7%; adjusted OR 1.25, 95% CI 1.22–1.29), more septic shock (21.6% vs 18%; adjusted OR 1.27), more acute kidney injury (40.6% vs 37.3%; adjusted OR 1.16), more adult respiratory distress syndrome (0.69% vs 0.57%; adjusted OR 1.17), and slightly more arrhythmias (24.7% vs 24.6%; adjusted OR 1.11). Teaching hospitals also had longer length of stay by 1.3 days and higher hospital costs by about $15,962.<br /><br />The authors concluded that adult patients hospitalized with sepsis in teaching hospitals had poorer outcomes, including higher mortality, longer stays, and greater costs. They note that further research is needed to understand why these differences exist and what factors may contribute to the worse outcomes observed.
