Diwerse: Diverse Identities and Workplace Experiences Reported in Setting of Healthcare and Education

The DIWERSE study examined perceived identity-based bias among physicians in a healthcare/education workplace using the Providers’ Experiences and Perceptions of Diversity, Equity, Inclusivity (PEPDEI) survey. The survey was sent to 429 faculty and resident physicians who had worked at the institution within the previous five years. It asked about verbal, physical, or emotional offenses related to identity, how often they occurred, who committed them, and whether they were witnessed.<br /><br />Of 276 respondents (64% response rate), 64.5% were trainees, 53.4% were women, 56.5% were White, and 42% identified as people of color (POC). The most commonly reported offenses were microaggressions (58%), hurtful or offensive language (47%), and inappropriate compliments (46%). Additionally, 30% said their suggestions were dismissed because of their identity, and 31% reported emotional or psychological distress. Faculty were frequently identified as offenders across several categories of identity-based insults. Reported experiences closely matched witnessed incidents, suggesting these behaviors were also observed by others.<br /><br />Women physicians were especially affected: 90% of White women and 95% of women of color reported at least one surveyed offense, compared with 50% of White men and 67% of men of color.<br /><br />The study concludes that women physicians and physicians of color experience significant identity-based offenses in the workplace, many of which are witnessed by others. The authors emphasize the value of bystander training and propose that the survey be used more broadly to monitor workplace climate, improve safety, and support better retention of faculty and trainees.
