Disproportionate Covid-19 Severity in Diverse Hospitalized Patients in New Mexico

This retrospective study examined 1,425 adults hospitalized with symptomatic, PCR-confirmed COVID-19 at the University of New Mexico Hospital from March 2020 to March 2021. Patients were grouped by self-reported race/ethnicity: American Indian/Alaskan Native (AI/AN), Hispanic, and non-Hispanic White (NHW), with other groups excluded from analysis.<br /><br />The study found clear disparities in outcomes. Although AI/AN patients were younger and had comorbidity profiles broadly similar to the other groups, they experienced more severe disease, longer hospital stays, more complications, and higher in-hospital mortality. In contrast, NHW patients had the highest rates of several pre-existing conditions such as hypertension, COPD, hyperlipidemia, hypothyroidism, and stroke.<br /><br />AI/AN ancestry was an independent predictor of both severe COVID-19 and death. Other predictors of severe illness included male sex, secondary bacterial pneumonia, ARDS, shock, cardiac failure, myocardial infarction, encephalopathy, and acute kidney injury. Predictors of mortality included AI/AN ancestry, ARDS, shock, cardiac failure, myocardial infarction, and acute kidney injury.<br /><br />The authors conclude that AI/AN hospitalized patients in New Mexico suffered disproportionate COVID-19 severity and mortality despite being younger and having comparable comorbidities, highlighting the need for targeted interventions to reduce racial and ethnic health disparities in COVID-19 outcomes.
