Disparities in Mychart Status During Transitions of Care

This study examined whether patient portal status at discharge (active vs non-active MyChart) was associated with patient demographics and post-discharge outcomes among 1,195 adults discharged from a general medicine unit at NYU Langone between November 2020 and October 2021.<br /><br />The authors found significant disparities in MyChart status at discharge by both age and race. Older patients were more likely to have non-active MyChart status, and MyChart activity also differed across racial groups, with lower active portal use among Black patients compared with White and Asian patients. Language was not significantly associated with MyChart status.<br /><br />Regarding outcomes after discharge, MyChart status was strongly associated with outpatient follow-up completion. Patients with active MyChart were much more likely to complete a post-discharge outpatient visit than those with non-active status. In contrast, MyChart status was not significantly associated with 30-day readmission rates.<br /><br />Overall, the findings suggest that patient portal use during transitions of care is uneven across demographic groups and may influence follow-up care after hospitalization. The authors conclude that disparities in MyChart status and outpatient visit completion indicate an opportunity to improve equity in transitions of care, particularly by better engaging older adults and minority patients through patient portals. Further research is needed to identify effective strategies to increase portal engagement and improve access to follow-up care.
