After surgery, the team transferred him to Hospital-at-Home on postoperative day 4. He continued to receive inpatient hemodialysis while recovering at home and was even able to work virtually. He remained in HaH until a community dialysis chair became available, which occurred 41 days later.<br /><br />The poster argues that perioperative medicine should do more than provide surgical “clearance.” It can identify system-level barriers to discharge early, helping reduce length of stay, lower costs, and improve patient flow. Hospital-at-Home is presented as a model that aligns with these goals by easing pressure on hospital beds, reducing readmissions and costs, and maintaining quality, safety, and patient satisfaction.
