This poster describes how a perioperative medicine clinic at Oregon Health & Science University helped reduce hospital capacity strain by proactively identifying a patient who was a candidate for Hospital-at-Home (HaH) after surgery.<br /><br />The case involved a 63-year-old man with polycystic kidney disease and bilateral renal cell carcinoma who underwent bilateral nephrectomy. Before surgery, his outpatient nephrologist arranged referral for community hemodialysis, but local dialysis chair wait times were 2–3 months. That delay created a major discharge barrier, since the patient would otherwise have had to remain in the hospital after surgery despite no longer needing acute inpatient care.<br /><br />During a pre-operative clinic visit, the patient was referred to HaH before hospitalization. After surgery, the team transferred him to Hospital-at-Home on postoperative day 4. He continued to receive inpatient hemodialysis while recovering at home and was even able to work virtually. He remained in HaH until a community dialysis chair became available, which occurred 41 days later.<br /><br />The poster argues that perioperative medicine should do more than provide surgical “clearance.” It can identify system-level barriers to discharge early, helping reduce length of stay, lower costs, and improve patient flow. Hospital-at-Home is presented as a model that aligns with these goals by easing pressure on hospital beds, reducing readmissions and costs, and maintaining quality, safety, and patient satisfaction.
