DEFINE HM is a new hospital medicine rotation at Yale New Haven Hospital designed to improve PGY-1 interns’ exposure to “fresh” ED admissions while addressing gaps caused by geographic team localization, high census, prolonged ED boarding, and discharge barriers.<br /><br />In this model, a dedicated, de-localized hospitalist admitting team pairs one attending with one PGY-1 learner. The dyad admits 3–5 patients daily over 12 days in a 2-week rotation, managing patients from the ED until an inpatient bed becomes available. The curriculum is case-based and emphasizes the initial diagnostic workup, triage, rapid intervention, medication reconciliation, systems-based practice, coordination with specialists, and clear communication.<br /><br />The study aimed to explore learners’ perceptions of the educational value of the rotation using qualitative methods. Focus groups were informed by learner narratives and included PGY-1 residents who completed the rotation. To date, one focus group with three participants has been completed.<br /><br />Early findings suggest learners found the experience highly valuable. Emergent themes included: acquisition and development of unique skills, individualized growth, meaningful interaction and opportunities for emulation, and mentorship/career exploration. Participants described gaining a better understanding of hospital systems and appreciating the chance to focus fully on admissions without floor responsibilities.<br /><br />Overall, embedding interns on a direct hospitalist admitting team appears feasible, sustainable, and potentially reproducible at other institutions, while offering a meaningful educational experience centered on undifferentiated patient care and professional development.
