Development of a Point-of-Care Ultrasound Track for Internal Medicine Residents
Back to course
Pdf Summary
The document describes a 3-year point-of-care ultrasound (POCUS) residency track for internal medicine (IM) residents designed to build basic competency and develop future leaders in POCUS education. The track aims to provide longitudinal training, help residents earn SHM-ACCP POCUS Certificate of Completion certification, and create POCUS champions who can expand institutional training capacity.

The program was created in response to growing demand for POCUS training in IM residency programs, along with evidence that longitudinal training improves knowledge and skill retention more than one-time workshops. A major challenge is that many programs lack enough trained faculty, so this track uses external educational resources to fill that gap.

Each year has structured components. Residents complete online modules, procedure rotations, POCUS CME courses, and an image portfolio, while also participating in peer-to-peer teaching of junior residents and medical students. Supplemental learning includes readings, online review questions, and practice during inpatient rotations.

Outcomes from 12 residents across all three years were very positive. Survey response was 100%. Residents reported increased use of and comfort with both diagnostic and procedural POCUS. All respondents would recommend the track, and all were satisfied or very satisfied with it. Every third-year resident successfully completed the SHM-ACCP POCUS COC program and felt it helped with job searches or fellowship applications.

The most common barriers to using POCUS were time constraints, lack of equipment, and lack of trained faculty supervision. The program addressed these issues by integrating external certification resources and by having residents teach others, which helped increase institutional teaching capacity.

Overall, the track is presented as a practical model for other IM residency programs that want longitudinal POCUS training but face resource or expertise limitations.
