Development of a Novel Observation Tool to Assess Medical Team Behavior in Addressing Social Drivers of Health on Inpatient Rounds

The project developed a novel observation tool to assess how inpatient medical teams address social drivers of health (SDoH) during rounds. Because SDoH strongly affect outcomes such as readmissions and mortality after discharge, the authors aimed to create a flexible way to measure team behaviors and identify educational needs or the impact of future interventions.<br /><br />The tool was built using the National Academies of Medicine, Engineering, and Sciences (NASEM) “5As” framework for integrating social care into medicine: Awareness, Adjustment, Assistance, Alignment, and Advocacy. A Delphi method was used to refine the tool. The pilot version includes 8 encounter-information items and 17 SDoH-related items focused on awareness, adjustment, and assistance. Alignment and advocacy were not included because they were rated below the threshold for reliable observation during rounds. A “free notes” item was also included despite being below the clarity threshold, because it could capture useful narrative details.<br /><br />Overall, the authors present this as a structured but flexible tool for observing social care behaviors in inpatient settings. Next steps include piloting the tool to evaluate interrater reliability and collecting baseline data.
