Novant Health, a system with 15 hospitals and 14 ambulatory surgery centers, had inconsistent preoperative evaluation processes across locations, including phone calls, nurse visits, and provider visits. To reduce variability and improve efficiency, anesthesiologists and hospitalists across markets collaborated to create a single universal preoperative medical questionnaire.<br /><br />The team reviewed perioperative literature and existing triage tools, then worked with health literacy experts to design a patient-facing MyChart questionnaire. Each question was assigned a weight, and the total produced a medical complexity score to support patient risk stratification. The MyChart Pre-Surgical Questionnaire launched in June 2022 across Novant markets, replacing previous questionnaires. In August 2022, the anesthesia pre-evaluation note was updated so providers could include nurse-validated questionnaire responses directly in the history section.<br /><br />The questionnaire is intended to engage patients in their own care, standardize assessment across sites, and streamline history taking. It also helps determine the appropriate level of preoperative evaluation earlier in the surgical process.<br /><br />Results are still being collected. The team plans to evaluate completion rates, whether patients or nurses complete the questionnaire first, and whether the questionnaire should be shortened or expanded. They will also study how medical complexity scores relate to procedural risk to better guide triage decisions between phone calls, nurse visits, and provider visits.<br /><br />Overall, the universal questionnaire aims to improve preoperative coordination, earlier risk identification, and efficient use of perioperative resources.
