This project developed an inpatient IV iron order set to promote high-value care and reduce use of expensive iron formulations, especially ferumoxytol. A multidisciplinary team of hospitalists, hematologists, pharmacists, and data scientists created an EPIC order set that embedded diagnostic criteria for iron deficiency anemia, recommended evaluation of underlying causes, and guided appropriate follow-up. Iron dextran was placed as the preferred IV iron option, with other formulations available as alternatives.<br /><br />The intervention also included provider education on evidence-based diagnosis and treatment of iron deficiency anemia, plus quarterly feedback on individual prescribing patterns. The goal was to improve awareness of cost differences between IV iron products and reduce unnecessary use of ferumoxytol, which is significantly more expensive than iron dextran, iron sucrose, or ferric gluconate.<br /><br />After the order set went live in April 2023, prescribing patterns shifted substantially. In 2022, only 19.6% of IV iron orders were for preferred non-ferumoxytol formulations. By 2024, 93.5% of orders were for non-ferumoxytol one-gram doses, representing a 91% increase in use of cost-conscious formulations. The authors estimate cost savings of at least $83,840 in 2024.<br /><br />Overall, the project showed that a structured order set combined with education and feedback can meaningfully improve IV iron prescribing, lower costs, and support more accurate diagnosis and workup of iron deficiency anemia.
