Development and Implementation of a Hospital Medicine Pa Post-Graduate Program

This document describes the development and early outcomes of a 12-month post-graduate Hospital Medicine Physician Assistant (PA) fellowship at Brigham and Women’s Hospital. The program was created to address limited onboarding and training options for advanced practice providers in hospital medicine, where broad and complex clinical knowledge is required.<br /><br />Designed by PA directors, a physician medical director, and a hospital administrator, the fellowship includes four 10-week core rotations in General Medicine, Cardiology, Oncology, and Critical Care, plus a 4-week elective. Training occurs across a quaternary care center, a community hospital, and an inpatient cancer center. Fellows work as responding clinicians alongside PA preceptors. The program also provides more than 300 hours of protected didactic education, over 25 hours of simulation, individualized mentorship, professional development, and a longitudinal capstone project. Fellows and preceptors complete post-rotation evaluations.<br /><br />Since its launch in 2021, eight fellows have participated. Survey results show strong perceived benefit: all 19 fellow post-rotation surveys reported improvement in clinical skills, knowledge, and professional growth. All six current or graduated fellows reported satisfaction with expectations, support, and feedback. Among 10 preceptor surveys, all respondents said they would precept again, 90% reported increased job satisfaction, and 60% felt more likely to stay in their role because of the fellowship.<br /><br />The program has also achieved strong retention, with 100% of graduating fellows remaining in PA positions within the institution. Overall, the fellowship has successfully created a structured postgraduate training pathway, improved PA competence, supported retention, and increased preceptor satisfaction. Based on early success, the hospital plans to expand the model to other departments.
