Developing an Emergency Response Simulation-Based Curriculum for Internal Medicine Residents

This document describes a simulation-based emergency response curriculum designed for internal medicine residents at a Veterans Hospital, where there were safety concerns about resident performance during rapid response and code blue events. The curriculum was created to improve residents’ knowledge, confidence, and ability to lead critical events.<br /><br />The training included a brief didactic PowerPoint and six simulated patient cases, delivered in two separate 2-hour sessions led by a chief resident. These simulations were run alongside training for hospitalists using a standardized rapid response script (ICRASHED). The simulation flow involved an introductory lecture, responding to an overhead rapid response call, gathering history and vitals, ordering tests and treatments, managing cardiac arrest, achieving return of spontaneous circulation (ROSC), and debriefing.<br /><br />Pre- and post-training surveys assessed residents’ knowledge, self-assessed preparedness, and attitudes. Results showed clear improvement after training. Before the curriculum, 70% of respondents felt unprepared to lead critical events even with supervision. After training, 80% felt prepared to lead a patient emergency while supervised, a statistically significant improvement. Knowledge also improved: correct identification of epinephrine dose and route increased from 76% to 94%, and knowledge of rhythms requiring defibrillation increased from 85% to 94%.<br /><br />The authors conclude that simulation training led to immediate knowledge retention and improved perceived preparedness for emergency leadership. They also emphasize the value of using patient safety data to guide educational curriculum design in order to address specific performance gaps and potentially improve patient outcomes.
